The precise mechanism of action of tianeptine remains unclear despite several decades of research and clinical use, and while it has been speculated that tianeptine might act as an allosteric modulator of the serotonin transporter at a separate site to that used by SSRIs, newer research suggests that its direct action is actually alteration of AMPA receptor AMPA glutamate receptor activity, which then leads to enhanced serotonin reuptake via a downstream mechanism which has yet to be fully characterised, but seems to involve altered neuroplasticity and release of BDNF . ref name pmid12372016 cite journal author Kole ... and anxiety , and are in marked contrast to other antidepressants such as the selective serotonin reuptake inhibitor s SSRIs , which Reuptake inhibitor inhibit the reuptake of serotonin instead. ref name ... 1. Allosteric serotonin reuptake inhibitor

Allosteric serotonin reuptake inhibitor is a marketing classification invented by the pharmaceutical company Lundbeck for its selective serotonin reuptake inhibitor SSRI escitalopram , the S stereoisomer of the SSRI citalopram . It is based on the observation that the R isomer of citalopram can decrease the potency and inhibit the effects of the S isomer, probably through an allosteric interaction between two distinct, non overlapping binding sites for the two different isomers on the serotonin transporter . The term allosteric refers to the negative interaction between the binding sites of to different drugs or isomers, however, it does not relate to the action of one drug to the inhibition of serotonin uptake. Allosteric serotonin reuptake inhibitors are therefore not a therapeutically distinct class of SSRI. Etymology There are several parts to the phrase endocannabinoid reuptake inhibitor. Firstly, a reuptake inhibitor is a substance the prevents it s respective neurotransmitters from reabsorbing into the pre sypnatic neurones, which makes them continually recycle themselves thus creating a large increase in neurotransmission. Next, a cannabinoid is simply a class of closely related substances such as Tetrahydrocannabinol and Cannabidiol . Endo is a prefix used to describe a cannabinoid that is naturally found within an animal. In retrospect, an endocannabinoid reuptake inhibitor is a substance that when ingested by an animal prevents it s natural cannabinoids from being reabsorbed into the neurones they come from. Pharmacology The inhibition of endocannabinoid reuptake raises the amount of those neurotransmitters available in the synaptic cleft and therefore increases neurotransmission . Following the increase of neurotransmission in the endocannabinoid system is the stimulation of its functions which, in humans, include suppression of pain perception analgesia , increased appetite , mood elevation and inhibition of short term memory . Use in medicine Other than toxicity research and recreational use, eCBRIs could have some potential in fighting tumors and possibly cancer . A study done in 2004 on rats with thyroid tumors showed that reuptake inhibition of the endocannabinoid system using VDM 11 and AA 5 HT reduced the ultimate size of the tumors in the treated rats. These findings suggest that the use of cannabinoids ...   more details

  1. Reuptake enhancer

Image Tianeptine.svg thumb right 175px Tianeptine , the prototypical reuptake enhancer. A reuptake enhancer RE , also sometimes referred to as a reuptake activator , is a type of drug which enhances the plasmalemmal transporter mediated reuptake of a neurotransmitter from the synapse into the synapse pre synaptic neuron , leading to a decrease in the extracellular concentration s of the neurotransmitter and therefore a decrease in neurotransmission . Tianeptine Stablon , a selective serotonin reuptake enhancer SSRE , is currently the only reuptake enhancer known. 1. Serotonin antagonist

A serotonin antagonist is a drug used to inhibit the action at serotonin receptor s. Many of the most important medications of this class selectively act at the 5 HT3 receptor , and thus are known as 5 HT3 antagonist s. However, other drugs such as ketanserin act upon other types of serotonin receptors. Antagonists of the 5 HT2A 5 HT sub 2A sub receptor are sometimes used as atypical antipsychotic s, with the typical antipsychotic s being purely dopamine antagonist s. Ketanserine blocks 5 HT2A,5 HT2C and alpha 1 receptors. Ondansetron,Dolansetron,Granisetron are 5 HT3 receptor antagonist.they are efficacious in treating chemotherapy induced emesis. Clozapine blocks 5 HT2A,5 HT2C,D4 receptors. Quetiapine blocks 5 HT2A,5 HT1A,D1 2,HI,alpha 1 receptors. Methsergide is 5 HT2A 2C antagonist and nonselective 5 HT 1 receptor blocker.it causes retrperitoneal fibrosis and mediastinal fibrosis. Cyproheptadine blocks 5 HT2A, HI and it is a mild anticholinergic.
