The effect of the antibodies and the immunological pathway is most likely either of three mechanisms Steric hindrance of the Desmogle n 1. The antibody caps off the site for intracellular binding to another keratinocyte. Activation of a endocytic pathway. The antibody activates a pathway which causes a internationalization of Desmogle n 1 which in turn causes a loss of adhesion. Disruption of function. In this case the antibody blocks the Desmogle n 1 from being formed into a desmosome. This in turn causes a loss of adhesion with acantholysis as a result. Fitzpatrick s Dermatology in General Medicine . 6th ed. . McGraw Hill. ISBN 0 07 138076 0. ref rp 561 Pathophysiology It is an autoimmune disease caused by antibody antibodies directed against both desmoglein 1 and desmoglein 3 resulting in the loss of cohesion between keratinocyte s in the Epidermis skin epidermis . It is characterized by extensive flaccid blisters and mucocutaneous erosions. The severity of the disease, as well as the mucosal lesions, is believed to be directly proportional to the levels of desmoglein 3. Milder forms of pemphigus like foliacious and erythematoses are more desmoglein 1 heavy. It arises most often in middle aged or older people, usually starting with a blister that ruptures easily. The lesions can become quite extensive. The pathogenesis of the disease involves autoantibodies against desmosome proteins, separating keratinocytes from the basal layer of the epidermis. On histology, the basal keratinocytes are usually still attached to the basement membrane leading to the appearance and thus the term, tombstoning . br Transudate Transudative fluid accumulates in between the keratinocytes and basement membrane suprabasal split , forming a blister. This is a contrasting feature from bullous pemphigoid , where the detachment occurs between the Epidermis skin epidermis and dermis subepidermal bullae . Diagnosis The gold standard for diagnosis is a punch biopsy of the lesion with direct immunofluorescence ... mediated damage to cell adhesion protein Desmoglein . br Pemphigus vulgaris is easy to confuse .... Refimprove date April 2009 File Cazenave, P. L. Alph e.jpg right thumb Pierre Louis Alph e Cazenave 1795 1877 Pierre Louis Alph e Cazenave 1795 &ndash 1877 was a French dermatologist who practiced medicine at the H pital Saint Louis in Paris . In 1823 he was appointed interne to the hospitals of Paris, and in 1835 became professor agr g to the medical faculty. Cazenave was a student of Laurent Th odore Biett , a physician who introduced into France an anatomical approach for analysis of skin disorders. This analytical method was first developed by two English physicians Robert Willan and Thomas Bateman physician Thomas Bateman . In 1828 with Henri douard Schedel, he published a work based on Biett s lectures and observations titled Abreg pratique des maladies de la peau . This compilation was to become a highly influential text regarding dermatology in the mid 19th century. From 1844 until 1852, Cazenave was editor of Annales des Maladies de la Peau et de la Syphilis , which was a journal dedicated to scientific dermatology. He is credited with coining the term lupus erythematosus , which he derived from Biett s symptomatic descriptions of the disease.
