    Diabetic myonecrosis is a rare complication of diabetes . It is caused by infarcted muscle tissue, usually in the thigh. Epidemiology The mean age at presentation is thirty seven years with a reported range of nineteen to sixty four years. The mean age of onset since diagnosis of diabetes is fifteen years. The female male ratio is 1.3 1. Other diabetic complications such as nephropathy , neuropathy , retinopathy and hypertension are usually present. Its major symptom is the acute onset muscle pain, usually in the thigh, in the absence of trauma. Signs include exquisite muscle tenderness and swelling. Investigations and diagnosis Tissue biopsy is the Gold standard test gold standard . Macroscopic ally this reveals pale muscle tissue. Microscopic ally infarcted patches of myocytes . Necrotic muscle fibers are swollen and eosinophilic and lack striations and nuclei. Small vessel walls are thickened and hyaline hyalinized , with luminal narrowing or complete occlusion. Biopsy cultures for bacteria , fungi , acid fast bacilli and stains are negative in simple myonecrosis. Creatine kinase is found to be normal. Erythrocyte sedimentation rate ESR is elevated. Planar X ray reveals soft tissue swelling and bone scan shows non specific uptake. Computed tomography CT shows muscle oedema with preserved tissue planes non contrast enhancing . It affects the lower limbs and is typically asymmetric. Proximal diabetic neuropathy typically occurs in aged type 2 diabetic males, though it may occur in younger diabetic patients and women too. Symptoms Muscle weakness and wasting most striking in the thigh and buttocks, but may also occur in the shin and foot Pain in the back or thighs Loss of knee reflex on the affected side and in the ankles Diagnosis Nerve conduction studies and needle electromyography are used to rule out other conditions and diagnose proximal diabetic neuropathy. Treatment Proximal diabetic neuropathy is very much reversible. This can be done by taking various measures. Controlling the blood sugar levels is the most important part of treating proximal diabetic neuropathy. Proper eating habits, medication, physical exercise, good mental health and avoiding harmful habits like drinking, smoking etc. all form a part of the lifestyle to control diabetes.     Other persons Leonard Thompson Leonard Thompson 1908 1935 is regarded as the very first person to have received injection of insulin as a treatment for diabetes . He received his first injection in Toronto , Ontario , on January 11, 1922, at 14 years of age. The first injection had an apparent impurity which was the likely cause for the allergy allergic reaction he displayed. After a refined process was promptly developed for extracting the insulin, the purified dosage was successfully delivered to young Leonard Thompson twelve days after his first dosage was administered. He then showed signs of improved health and went on to live thirteen more years taking doses of insulin.
